Required Medical Records
If you are a patient, physician, or healthcare provider interested in our services, please send us the patient's medical records containing the following:
The name, licenses, and qualifications of the patient's primary physician.
The physician's acknowledgment about compliance requirements and commitment to monitoring the patient during treatment.
The patient's demographic information.
Diagnosis reports.
A form consenting to MHC's real-world data collection policy.
